This dialogue webinar explores what is truly new in ESKD care: a shift toward individualized, goal-directed, high-quality treatment shaped by what matters most to each person. Through practical discussion and real-world cases, this webinar de-mystifies new concepts, including shared decision-making and person-centered care, and examines how approaches like incremental peritoneal dialysis (PD) can better align dialysis with patients’ clinical needs, life priorities, values, and hopes. Participants will explore who may benefit from home therapies across the ESKD journey, including older adults and people living with frailty, and what supports are needed to make home a viable and sustainable option.
