

{"id":2814,"date":"2020-03-25T16:37:16","date_gmt":"2020-03-25T21:37:16","guid":{"rendered":"https:\/\/ami.advancedrenaleducation.com\/wparep\/?post_type=article&#038;p=1192"},"modified":"2025-05-09T13:53:48","modified_gmt":"2025-05-09T18:53:48","slug":"hemodialysis-access","status":"publish","type":"article","link":"https:\/\/ami.advancedrenaleducation.com\/wparep\/asiapacific\/article\/hemodialysis-access\/","title":{"rendered":"Introduction to Hemodialysis Access"},"content":{"rendered":"<p>More than 90% of dialysis patients in the United States rely on chronic hemodialysis (HD) for renal replacement therapy<sup>1,2<\/sup>. A standard hemodialysis treatment includes the continuous processing of approximately 200-600 mL of blood per minute over a 3-4 hours period and is performed on a 3 times per week basis<sup>3<\/sup>.\u00a0 This treatment requires safe and readily available access to the circulatory system.<\/p>\n<p>The primary goal of chronic HD vascular access is to provide repeated access to the circulation with minimal complications<sup>1,4,5<\/sup>. A well-functioning vascular access is essential for patient survival and should have the following characteristics<sup>6<\/sup>:<\/p>\n<ul>\n<li>Safe\n<ul>\n<li>Few complications<\/li>\n<\/ul>\n<\/li>\n<li>Reliable\n<ul>\n<li>Provide repeated access to circulation<\/li>\n<li>Provide continuous blood flow of 400-600 mL\/min<\/li>\n<li>Long lasting<\/li>\n<\/ul>\n<\/li>\n<li>Simple\n<ul>\n<li>Easy to place<\/li>\n<li>Easy to use<\/li>\n<\/ul>\n<\/li>\n<li>Acceptable to patient\n<ul>\n<li>Painless<\/li>\n<li>Cosmetically acceptable<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>A well-functioning arteriovenous fistula (AVF) comes closest to meeting the above characteristics. It is created by the anastomosis (surgical connection) of a vein and artery, typically the radial artery and cephalic vein in the forearm. The AVF is the most reliable form of permanent vascular access and has the fewest number of complications<sup>1,4,5<\/sup>. For this reason, the AVF is considered to be the access of choice.<\/p>\n<p>In addition to the AVF, three other forms of chronic HD vascular access are in use:<\/p>\n<ul>\n<li>Prosthetic bridge graft or AV graft (AVG):\u00a0 A surgical connection of an artery to a vein using a synthetic tube. \u00a0They are most commonly placed in the arms or thighs and are another option for permanent hemodialysis access <sup>1,6<\/sup><\/li>\n<li>Central venous catheter (CVC): A catheter is placed through the skin into the jugular, femoral, or subclavian vein. CVC are typically used for temporary access and are not preferred for long-term access due to high rates of infection and thrombus <sup>1,6<\/sup>.<\/li>\n<li>AV shunts: A surgically placed external cannula composed of a U-shaped silastic tube with Teflon tips (eg, Quinton-Scribner or Thomas Shunt) that connect an artery and vein.\u00a0 Although rarely used today, they were the original \u201cpermanent\u201d vascular access for HD<sup>1,7<\/sup>.<\/li>\n<\/ul>\n<p>Creation and maintenance of a functional AVF will remain one of the greatest challenges in the dialysis field<sup>7\u20139<\/sup>. Despite the fact that multiple studies have demonstrated the AVF\u2019s overall superiority of clinical outcomes\u2014including lower rates of thrombosis, infection, septicemia<sup>1,4,5,10<\/sup> \u2014there is a wide global variation in vascular access utilization. This variance has been attributed in varying degrees to patient characteristics, surgical practice patterns, delivery systems, available technologies, and practitioner and patient bias<sup>8,11<\/sup>.
