The US prevalence of 8.9\u20139.3% among patients with CKD and\/or heart failure<sup>1<\/sup>.<\/p>\n<h4>Causes of Hyperkalemia in ESRD<sup>2\u20137<\/sup><\/h4>\n<p>Multiple factors are responsible for determining serum K<sup>+<\/sup>\u00a0concentrations in patients with ESRD, including the individual patient\u2019s diet, the dialysis prescription, medications, and other conditions or comorbidities. Some of the factors are listed below:<\/p>\n<ul>\n<li>Prolong fasting<\/li>\n<li>Excessive dietary potassium intake<\/li>\n<li>Marked catabolism<\/li>\n<li>Rhabdomyolysis<\/li>\n<li>Insulin deficiency<\/li>\n<li>Mineralocorticoid deficiency<\/li>\n<li>Medications<\/li>\n<li>Dialysis induced hyperkalemia (rare)\n<ul>\n<li>High dialysate potassium concentration<\/li>\n<li>Hemolysis<\/li>\n<li>Accidental potassium infusions<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h4>Symptoms:<\/h4>\n<p>Clinical symptoms related to hyperkalemia are nonspecific and most often, asymptomatic. However, hyperkalemia affects cardiac and neuromuscular cells which are particularly sensitive to changes in serum [K]. In patients with mild to moderate hyperkalemia, patients usually complain of muscle weakness, fatigue, paresthesia\u2019s, palpitations, and cardiac arrythmias. As evidenced by characteristic changes in the electrocardiogram (ECG) that serve as indicators of potential life-threatening arrhythmias, the first sign of increased serum [K] is tenting of the T wave. ECG changes may progress rapidly in conjunction with serum [K] levels. ECG changes include widening of the QRS complex, progressive development of atrioventricular conduction blocks, slow idioventricular rhythm, an ECG tracing that looks like a sine wave, ventricular fibrillation, and finally asystole<sup>8<\/sup>.<\/p>\n<h4>Management <sup>9<\/sup><\/h4>\n<p>The treatment strategies are designed to intervene at 3 different levels:<\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>Effect on the cell membrane polarization\n<ul>\n<li>Calcium: Stabilizer of cardiac cell<\/li>\n<\/ul>\n<\/li>\n<li>Movement of extracellular potassium into the intracellular compartment\n<ul>\n<li>Insulin<\/li>\n<li>Albuterol<\/li>\n<li>Sodium Bicarbonate (in acidosis)<\/li>\n<\/ul>\n<\/li>\n<li>Removal of potassium from the body\n<ul>\n<li>Potassium binding resin with sorbitol<\/li>\n<li>Hemodialysis:\n<ul>\n<li>Removes 70 to 150mEq of potassium<\/li>\n<li>Used as gold standard when other treatments fail<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<div class=\"vcex-spacing\" style=\"height:30px\"><\/div><div class=\"vcex-module vcex-divider vcex-divider-solid\" style=\"width:100%;margin-top:20px;margin-bottom:20px;border-top-width:1px;border-color:#dddddd;\"><\/div>\n<h4>References:<\/h4>\n<ol>\n<li>Mu F, Betts KA, Woolley JM, et al.
